The University of Guelph is committed to the health and wellness of our community members. In response to the COVID-19 pandemic, the University is currently restricting access to all our facilities and locations including but not limited to main campus (“University Locations”).
In order to assist the University in protecting our community members, the University of Guelph has developed a COVID-19 Daily Screening Tool, Vaccination and Exemption System and Rapid Test Submission Tool (together called “Mandatory Vaccination and Screening”) which includes a link to the Ontario government’s COVID Self-Assessment Tool (“Assessment Tool”).
Individuals including children, wanting to attend any of the University Locations MUST complete the Daily Screening Tool every day PRIOR to coming to any of the University Locations.
Each individual completing the Daily Screening Tool is asked to complete the Assessment Tool. If the result of using the Assessment Tool indicates that an individual seems to have symptoms of COVID-19 or should self-isolate, that individual must NOT come to any of the University Locations, with the exception of students, staff and faculty attending a scheduled appointment at the Student Wellness Services COVID-19 testing clinic. A health care provider should be consulted regarding any medical questions.
Anyone who attends the Guelph Campus or other University of Guelph work location without completing Mandatory Vaccination and Screening, or who does NOT comply with any of the University’s Mandatory Vaccination Policy, the University’s vaccination or testing or other COVID-19-related safety practices or requirements, or who falsely reports any information provided in the Mandatory Vaccination and Screening the results of the Assessment Tool, or who does not comply with the University’s COVID-19 safety practices will NOT be permitted at any University Locations and may be subject to such other action as is appropriate in the circumstances.

Collection, Use and Disclosure of Personal Information
In completing Mandatory Vaccination and Screening, you will be required to provide personal information including your name, email address, phone number, and your vaccination status. You may also be required to provide personal information regarding, as applicable, your COVID-19 test result, or information related to an exemption request or proof of completion of an educational session. This personal information will be collected by the University pursuant to the University of Guelph Act, 1964 and consistent with Ontario’s Freedom of Information and Protection of Privacy Act. Personal information will be retained for at least one (1) year and otherwise for a reasonable period of time given the purposes for which it was collected. The personal information will be used by the University to determine whether you have complied with its mandatory vaccination requirements. The information you provide in the Assessment Tool is not collected by the University. If an outbreak of COVID-19 takes place which may affect you, your personal information may be disclosed to the Wellington-Dufferin-Guelph Public Health or other applicable Public Health Unit to assist with contact tracing efforts or as otherwise required by law. Personal information collected by the University through Mandatory Vaccination and Screening may be aggregated on a fully anonymized basis such that it is no longer personal information and such aggregated anonymized information may be shared by the University with the Ministry of Colleges and Universities and our community and stakeholders in relation to the University’s response to the COVID-19 pandemic including the University’s reinforcement of existing public health measures and our own efforts to keep our community safe. By using any Mandatory Vaccination and Screening tool, you are consenting to the collection and storage of your personal information and sharing of such information as set out above.
